CONFLICT OF INTEREST FORM
INTENT TO COLLABORATE FORM
This form is required to be completed and included with proposals which include funding for subcontracts. A separate form is required for each of the individual subcontractors.
PI ASSURANCE FORM
This form takes the place of the assurance certification requirements found in the 398 budget form.

TRANSMITTAL FORM
This form is required to be completed by the GUMC submitting department and attached to each proposal. Proposals cannot be submitted to the sponsoring agency without a completed and approved document.
